Acceptance of my entry in these events is without assumption of the responsibilities of any kind by the USTA, the host club, committees of the management of any event in which I may enter or participate. In consideration of the acceptance of my entry, I do hereby for and on behalf of myself and my heirs and legal representatives release and forever discharge USTA, any host club, their officers, committees and representatives and their successors and assigns, of and from any and all claims and demands of every kind, nature and character which I may have or which may be suffered or sustained by me in connection with my activities during the period traveling to or from such event and I covenant not to sue therefore.
I understand that this tournament will be governed by applicable USTA rules and regulations, the rules and regulations to this tournament, the rules and procedures governing discipline of the players in USTA sanctioned tournaments, the USTA Code of Ethics, tournament policy and ranking regulation, I agree to conduct myself accordingly. I also understand that photographs will be taken during the tournament and my picture may be used for publicity, promotion and advertising. By entering the tournament, I hereby grant The Chico Racquet Club & Resort the right to use these photographs.
